Sleeping with feet towards the door

Question:

Is there a clear source for the Minhag/Issur to sleep with feet facing the door?
If there is substance, then is a cupboard or armchair or suitcase etc between the bed and door considered a problem?
Is there a difference if a person is sleeping on a bed or the floor?
If the bed is a bit angled away from the door?
Some people quote Rav Belsky z”l to be lenient, and I’ve attached the halacha sheet. He is quoted to having said if a person isn’t lying “as if he is about to be taken out” then it’s not in the chashash. What does that mean?

Answer:

I have spoken on numerous occasions with the Chevrah Kadisha here and they told me that they always take out a niftar feet first. Maybe Rav Belsky meant that the person is lying directly in the doorway feet first.
