Like every contract, a person should understand a ketuba before signing on it. However (and also like other contracts), even if he does not understand it, his signing the ketuba, and the accompanying kinyan that is made, are binding.
There is no need to translate the ketuba, but the basic obligations therein should be explained to the chasan.
