Does one need kavanah before doing any mitzvah like tzedakeh and chesed or learning torah or any of the six constant mitzvos. It is difficult to have kavanah constantly.

One should certainly have kavanah when he does a mitzvah that he is doing a mitzvah of Hashem, this is generally automatic when doing a mitzvah act such as tzedakka and chesed.
The 6 constant mitzvos obviously don’t require that a person think about these 6 things every second, as he wouldn’t be able to do anything else! Rather they refer to a latent awareness that a person should always live with. This awareness is gained by setting aside time to learn about these mitzvos and contemplating them, which will affect his awareness throughout the day.
