Hello,
Essentially you have to say Shemona Esrei again[1], however there are two exceptions:
- If you remembered during Shemona Esrei that you have to say Hamelech Hakadosh, and[2] the doubt that you didn't say it only came up a while after finishing Shemona Esrei. Then we will assume that you did say it and you just don't remember that far back. Or on Rosh Hashana[3] or Yom Kippur, and you remember that you started saying, "v'sain ten pachdicha", then we can assume that you continued saying correctly, which would include Hamelech Hakadosh.
