Hi Rabbi,
Several years ago, I made a large order for pickup at a kosher store that was further away. They accidentally delivered an extra of one item. I called them to let them know and they told me to bring it back next time I came and I said ok. I didn't go to that store very often in person. I don't remember exactly but I think either the item was lost or accidentally used by another relative. I then tried to pay the store by calling them and leaving a message and asking them to charge me for that without sending the item on their order form. I think they may have been confused but I never heard back. What is my obligation at this regarding this item.

Being that you are not sure if you paid them back or not, and they also don’t know, they wouldn’t have a v\clear claim against you, however "לצאת ידי שמים", you should pay them for it.
