Yes, one says it each time they visit to comfort the mourners. The idea of saying Hamakom Yinachem eschem is that we are telling the mourner that, essentially, it is not possible for us to change the person’s emotional loss over their loved one. However, Hashem (Hamakom) makes a miracle, and He will find the way to comfort the person in their loss. This blessing that the comforter gives to the mourner, in a way, activates this bracha. Therefore, each time it is said, it adds to the bracha for the mourner.
