Having a one-on-one therapy session with a woman is a very serious yichud issue. Aside from the regular yichud risk, a therapy session involves very personal conversations and is also high risk regarding yichud. Offhand, there are a few options, though they might not be practical for you. One option is that the secretary will walk in in the middle of the session (which is highly unlikely). Alternatively, the door is left slightly open so that anyone can walk in from the street in the middle of the session. Another option is that there is a live video of the two of you that can be seen by someone in real time.
