If I have ice in the shape of a specific item, like a star, heart or letter, is it permitted to put those pieces of ice into a drink on shabbos when I know they will melt? If permitted, may I also put the ice in my mouth and back into the drink?

It is permitted to put the ice cube into the water on Shabbos, even though it has a specific shape. The fact that it has a specific shape which will get erased when it melts is not considered mochek. The reason is because the melacha of mochek only applies to writing that is on top of something else, and not when the actual item is a letter or shape. It is for this reason that it is permitted to eat an “animal cookie” “Alpha-bits”, letter shaped macaroni, a flower cookie, or to take a piece of fruit that is shaped like a swan.
Regarding your second question, I don’t the difference if the ice was in your mouth first or in the drink first.
