Am I required to give maaser from the pain and suffering award?

Yes you should give maaser from the award money. There isn't a specific amount of income that has to be earned to giving 20% or 10%, rather if the person can afford to give 20% it is a bigger mitzvah, and the segula that it will make the person rich is much stronger .
